About The Position

The Mental Health/Substance Abuse LPC will be responsible for providing mental health and substance abuse treatment services in an outpatient facility. Provide services within the following settings: IOP, PHP, and Ambulatory services. Provide clinical assessment and psychotherapy services for a defined patient population, where services are provided to patients, families, and/or groups. Position provides treatment in one or more of the following areas: substance abuse, emotional and behavioral issues, eating disorders, and other mental disorders outlined in the ‘Diagnostic and Statistical Manual of Mental Disorders’ (DSM).

Requirements

Master’s degree or higher in the mental health field.
Must possess a license from the State of Tennessee Mental Health Professions Licensing Board that allows for the independent practice of Clinical Services.
Must maintain a professional license and stay current (not revoked, suspended, or lapsed in registration), valid and unrestricted.

Responsibilities

Clinical assessments, treatment, case management, consultation, referral and education according to the individual’s professional discipline.
Services shall be provided in accordance with the established Tennessee Mental Health and Substance Abuse Standards as well as within the scope of practice of the individual.
Supervise all students, BSW’s, and MSW’s.
Conduct psychosocial evaluations and provide therapeutic interventions such as direct psychotherapy, including crisis counseling to individuals, groups, and families as needed to facilitate.
Coordinate all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.
Monitor patients on an ongoing basis to continually assess needs and facilitate treatment and interventions.
Maintain case records to reflect course of treatment and provide required documentation.
Document clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient care.
Evaluate the individual and family systems as well as assess a client’s functioning within work, family, and routines of daily living and identify areas needing continued support, resources, and treatment in order to assist clients.
Provide education and orientation for patients, families, and others assigned to treatment program to aid in therapeutic process.
Collaborate with the patient in the development of a written or electronic treatment plan.
Include patient and family involvement in assessment, treatment, education, and discharge planning.
Develop, implement, and maintain partnerships with other community providers.
Provide follow-up and aftercare planning services as needed.
Coordinate with community-based providers to assure optimal levels of care post discharge and to maximize treatment outcomes.
Actively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with physicians and peers.
Attend staff meetings, supervisory conferences, and other activities, which ensure the smooth functioning of clinical operations.
Accurately code counseling sessions, group sessions, and all other counseling involving IOP/PHP
