About The Position

Deer Oaks Behavioral Health is seeking a part-time Licensed Professional Counselor (LPC) to join its integrated clinical team serving residents in one or more local long-term care facilities. The role provides therapy, assessment, care planning, and behavioral health support to older adults and individuals with chronic medical conditions. The LPC works onsite and has professional autonomy, a predictable Monday–Friday schedule, and administrative and clinical support.

The position includes individual, family, and group therapy; clinical assessments and treatment planning; brief, goal-oriented interventions; and support for concerns common in long-term care settings. The counselor collaborates with facility staff and other providers, maintains timely documentation, and follows ethical, professional, and regulatory requirements. Deer Oaks has focused on behavioral health in long-term care since 1992 and serves skilled nursing and assisted living facilities nationwide.

Requirements

• Active, unrestricted Licensed Professional Counselor (LPC) license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfort navigating long-term care environments.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to long-term care residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents experiencing depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in long-term care.
• Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
• Coordinate treatment and work toward improved resident outcomes with nursing, social services, rehabilitation, and medical providers.
• Participate in care plan discussions, facility meetings, and case collaborations as needed.
• Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
• Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery.
• Build professional relationships with facility staff and leadership and serve as a behavioral health resource.
