About The Position

The Mind Body Wellness Center is seeking a Licensed Professional Counselor or Licensed Clinical Social Worker to join their team. This role involves assessing the social and emotional factors affecting patients' well-being, providing direct social work and counseling services, and connecting patients and families with necessary resources. The position also requires managing caseloads, maintaining accurate records, collaborating with healthcare providers, and participating in professional development. The role may involve exposure to chemicals and requires regular travel with reliable transportation.

Requirements

Bachelor’s or Master’s degree in social work from an accredited institution.
At least one year of social work experience, preferably in a medical setting.
Must be licensed as a Professional Counselor or Clinical Social Worker.
Must have access to a valid driver’s license in Pennsylvania.
Proof of insurance.
BLS Certification for basic life support.

Responsibilities

Evaluate how illness affects patients' social, emotional, and psychological well-being.
Provide support to patients and their families through counseling and social work services.
Connect patients and families with available resources for social, emotional, and practical support.
Help with financial or environmental challenges as necessary.
Manage and track the productivity and status of patients.
Maintain accurate patient records, including progress notes and reports.
Work with physicians and other healthcare providers to address social and emotional factors affecting health.
Provide guidance to external agencies or internal departments on social and emotional factors affecting patients.
Ensure accurate communication with both internal and external stakeholders.
Participate in workshops and professional meetings to stay updated in the field.
