About The Position

Elara Caring is hiring a Licensed Practical Nurse (LPN) for its Personal Care Services team in the Jefferson City, Missouri branch, serving Jefferson City, Columbia, and surrounding areas. The role provides nursing care to patients in their homes and supports Elara Caring’s mission to deliver care in patients’ preferred home settings. The position offers a $2,500 sign-on bonus.

The LPN delivers care in accordance with patient care plans, monitors and communicates patient status, supports medication management, and educates patients and families. The nurse collaborates with RN Case Managers, physicians, and other members of the care team. The role reports to the Clinical Manager or Branch Director for Personal Care Services and requires travel, including reliable personal transportation.

Requirements

• Current LPN/LVN license in the state of Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care.
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Compassionate, patient-first mindset.
• Reliable personal transportation; travel is required approximately 50%.
• Ability to meet health screening and compliance requirements.

Nice To Haves

• Home care, home health, or hospice experience is preferred.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow the Plan of Care before each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and the care team.
