About The Position

Grand Itasca Clinic & Hospital, a part of Fairview Health Services, is seeking an enthusiastic LPN/MA to join their team. LPNs/MAs work closely with providers to deliver quality patient care and service by maintaining patient flow, escorting patients, identifying patient needs, collecting/verifying information, administering medications, updating electronic medical records, implementing methods to improve care and patient experience, maintaining exam/treatment rooms, ordering supplies, and displaying creativity while working effectively as a team member. This position is for a Licensed Practical Nurse (LPN) with provider Rourk, working .8-.9 FTE (64-72 hours per pay period) in the Clinic Nursing department.

Requirements

  • Current LPN Minnesota state license, current Medical Assistant certification or registration required. Must maintain license/certification/registration.
  • BLS certification to be completed within two months of employment.
  • Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.
  • Maintains reliable and consistent attendance.

Nice To Haves

  • Experience in an ambulatory health care setting is preferred.
  • Ability to work efficiently, accurately, and maintain positive working relationships.

Responsibilities

  • Conducts a focused assessment through the collection and comparison of data to normal findings and the patient's current health status, reporting changes to the appropriate licensed health care provider.
  • Participates with the health care team in the development and modifications to the patient's health plan of care/orders.
  • Implements patient care interventions delegated, ordered, or prescribed by a licensed health care provider.
  • Assigns nursing activities or tasks to other licensed practical nurses (LPNs).
  • Assigns and monitors nursing tasks or activities delegated to certified medical assistants or any unlicensed personnel on the health care team.
  • Focuses on patient care through the provision of safe/effective care, patient advocacy, assistance with patient evaluation to interventions, and collaboration/communication with health care team members.
  • Provides health care information to patients and reviews education materials.
  • Provides input for policies and procedures.
  • Accountable for the quality of care delivered, recognizes the limits of practice and competency, and performs to the level of education, knowledge, and skill ordinarily expected of individuals who have completed an approved LPN education program.
