About The Position
RiseWell Community Services is seeking a Licensed Practical Nurse (LPN) to join our Clinical Department at the Methadone Clinic. In this role, you will be responsible for providing health management and counseling as part of a treatment team for individuals with substance abuse disorders, in collaboration with a psychiatrist and registered nurse. You will ensure that medical standards are met according to program regulations.
Requirements
- Currently licensed as a Licensed Practical Nurse by the New York State Education Department
- A valid and clean NY State driver license
- Fingerprinting, criminal record check, approval from OASAS
- Good computer literacy skills

- Ability to utilize a team approach to problem solving

- Maintain licensing credentials
Responsibilities
- Medication management, education, and administration
- Attend assigned meetings
- Conduct all assigned visits
- Oversee the accurate completion of all documentation
- Maintain compliance with incident reporting guidelines
- Comply with agency Compliance Plan and False Claims Policy by preventing, detecting, and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state, and local regulations applicable to this job description.
- Proficiency in all agency-based IT applications
- Provide education to clients and staff regarding standards of medical care
- Perform all other job duties as assigned
