About The Position

The Licensed Practical Nurse provides professional nursing care to patients in the clinic/outpatient setting in compliance with professional practice standards. The role promotes a collaborative, interdisciplinary approach among patients, families, and all members of the care delivery team and is essential to the functioning of SageWest Medical Group. Workload is generated by the number of patients the provider encounters each day. The position requires the ability to make decisions and judgments based on knowledge and experience.

The position is PRN (as needed), with hours listed as 8:00 a.m. to 5:00 p.m. SageWest Health Care is located in Fremont County, Wyoming, and provides a range of healthcare services through hospitals in Riverton and Lander.

Requirements

• Maintain a current Licensed Practical Nursing license in Wyoming.
• Graduate of an accredited Licensed Practical Nursing program.
• Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient’s needs.
• Current BLS certification by the first day or completion as assigned by the Learning Center.

Responsibilities

• Implement and evaluate patient care and participate in developing patient-specific care plans.
• Provide direct patient care, including administering prescribed medications and treatments.
• Document patient care in healthcare records according to established standards.
• Delegate, prioritize, request and accept guidance and assistance to help ensure safe and timely care.
• Maintain professional effectiveness through continuing education and professional development.
• Assist with transporting patients, specimens, supplies, and equipment, and maintain and stock supplies and equipment.
• May triage patient phone calls, counsel and educate patients, take patient histories, administer prescribed medications, change dressings, clean wounds, and monitor vital signs.
• Perform other duties as assigned.
• Maintain a positive, cooperative, self-motivated, courteous, and professional attitude, work collaboratively with others, and respond appropriately to constructive feedback.
