About The Position

Elite Home Health is a Medicare-certified home health agency serving communities across Florida. We are seeking dedicated Licensed Practical Nurses (LPNs) committed to clinical excellence, patient-centered care, and supporting safe, compliant care delivery in the home setting. The LPN provides skilled nursing care to patients in the home setting under the supervision of a Registered Nurse (RN), delivering care in accordance with physician orders, established care plans, agency policies, and applicable state and federal regulations. This role supports patient care through direct skilled services, monitoring, documentation, and communication with the interdisciplinary care team.

Requirements

  • Graduate of an accredited Practical Nursing program required
  • Current Florida Licensed Practical Nurse (LPN) license in good standing required
  • Minimum of one (1) year of nursing experience required
  • Current CPR certification required
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to pass background screening and all compliance requirements

  • Home health, hospice, or acute care experience preferred

Responsibilities

  • Provide skilled nursing care under the supervision of an RN
  • Deliver care in accordance with physician orders and established care plans
  • Monitor patient condition and report changes to the RN and care team
  • Support patient comfort, safety, and continuity of care
  • Perform skilled nursing procedures within LPN scope of practice and agency policy
  • Administer medications and treatments as permitted by state regulations and competency validation
  • Provide wound care, dressing changes, catheter care, and related nursing tasks
  • Assist with rehabilitative and supportive care interventions
  • Observe and report changes in patient condition, response to treatment, or concerns
  • Communicate relevant updates to supervising RN and care team
  • Reinforce patient education provided by RN or care plan
  • Complete timely and accurate clinical documentation in the EMR system
  • Document patient care, observations, and interventions performed
  • Follow Medicare requirements, state regulations, and agency policies for documentation
  • Participate in coordination with RN and interdisciplinary care team
  • Follow established care plans and notify RN of any concerns or changes
  • Participate in required training, in-services, and staff meetings
