About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency serving nine Western Pennsylvania counties. The Licensed Practical Nurse joins its home health team to provide skilled nursing care and support to patients in their homes, while helping meet each individual's needs with respect, dignity, and compassion. The role involves connecting with patients and their families, supporting them through their health journey, and working autonomously while collaborating with other healthcare professionals. Hospice experience is preferred but not required. Benefits and wages may vary by location, and certain benefits may require minimum weekly hours or employment status.

Requirements

• At least one year of LPN experience.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Compassion, empathy, and emotional resilience to support patients during end-of-life situations.
