The Ohio State Wexner Medical Center Monday–Friday 8:30 AM -5:00 PM (On call requirements) Looking for More Independence and Meaningful Patient Care? Join our Home Health team and experience the rewarding opportunity to provide one-on-one care while helping patients recover safely and comfortably in their own homes. As a LPN, you'll work independently while being supported by an experienced clinical team. You'll build meaningful relationships with patients and families, make a direct impact on patient outcomes, and enjoy variety in your day-to-day work. Post-acute care is at the center of improving health outcomes. We care for patients where they spend the majority of their time – in their homes. This privileged position allows us to see things that are invisible to a patient’s primary care or hospital physician, and to deliver the best possible care tailored to each patient’s setting. As the healthcare industry continues to evolve away from hospital-centric care, our work, caring for patients in their homes becomes more important than ever. Our success helping patients recover comes from engaging with them. We start at the facility bedside and coordinate each patient's transition back to their home, ensuring a smooth transition and that their care plan is right for them. We support complex, high-acuity, medically-at-risk aging patients. This is the most challenging group of individuals to serve, and we find it the most rewarding. And we do this all in partnership with health systems. Together we create a seamless care environment that enables patients to receive excellent care in the setting that best meets their needs. Employees of The Ohio State University Wexner Medical Center Home Care are employed by Alternate Solutions Health Network, LLC. Speech Language Pathologists are directly employed by Alternate Solutions Homecare of Columbus, LLC.
Requirements
- Current Licensed Practical Nurse (LPN) license
- Minimum one year of acute care experience required
- Valid driver's license and active auto insurance
- Ability to travel throughout the assigned territory
Nice To Haves
- Home health experience preferred
- Strong assessment, communication, and critical thinking skills
Responsibilities
- Provide skilled nursing care in patients' homes under physician orders and RN direction
- Assess and monitor patient conditions
- Administer medications and treatments
- Educate patients and families on disease management and preventative care
- Collaborate with physicians, case managers, and interdisciplinary team members
- Complete timely clinical documentation in accordance with agency and Medicare guidelines
- Help reduce hospital readmissions through patient education and proactive care
