About The Position

AHRC New York City is seeking Licensed Practical Nurses (LPN) to fill two part-time overnight shifts at their ICF residence in Queens. The LPN will be responsible for monitoring the health and safety needs of individuals living in the residence to promote their overall wellness. This role involves supervising and participating in direct support staff tasks, ensuring adherence to medical and nutritional guidelines, administering medications, and maintaining infection control practices. The LPN will also be responsible for documenting medical findings, communicating with medical providers and families, and providing various medical treatments and interventions as ordered. Additionally, the LPN will monitor vital signs and weights, ensure compliance with regulations, maintain medical charting, attend medical appointments when possible, and follow emergency procedures. Coverage in accordance with staffing schedules and completion of necessary training are also required.

Requirements

Current NYS Licensed Practical Nurse Licensure required.
AHA or equivalent CPR certification.
Excellent written and verbal communication skills.
Ability to handle multiple priorities and work as a member of a team.
Must possess the ability to deal tactfully with personnel, people supported, family members, visitors, government agencies, and the general public.
Must have the ability to implement the programs goals, objectives, policies, procedures, etc. of the Residential Nursing Services Department.
Must maintain the oversight of care and use of supplies, equipment, etc. and maintain the appearance of the site.

Nice To Haves

Experience with people with intellectual and developmental disabilities is preferred but not required.
Valid NYS Driver’s license highly preferred.

Responsibilities

Supervise and participate with direct support staff in tasks related to personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness.
Supervise and participate with direct support staff in mealtime and feeding protocols, ensuring adherence to recommended nutritional and medical guidelines.
Perform medication administration as per procedure outline.
Report and oversee infection control practices as outlined per agency and per program-specific exposure control plans.
Oversee and ensure adequate basic First Aid supplies are available.
Report and document medical findings for RN review and for inclusion to a person’s plan of nursing services.
Report immediately any significant deviations in the medical status of a person supported to RN Supervisor.
Communicate, report, and document issues and concerns of the person supported to primary medical provider and RN Supervisor.
Communicate with families and maintain family contacts to inform and discuss pertinent medical concerns of people supported.
Provide post-operative, routine, or preventative medical treatments and interventions as per MD or RN order (e.g., G-tube feedings, wound and skin care, urinary drainage systems maintenance, blood glucose monitoring, passive and active range of motion therapy).
Obtain baseline and monitor medical information (Vital Sign Monitoring, Weights).
Ensure ongoing compliance and quality assurance with Federal and State regulations pertinent to the operation of the residence.
Utilize appropriate and expected medical charting skills.
Attend medical appointments as requested by RN Supervisor as individual LPN scheduling permits.
Observe and follow through on established emergency notification procedures.
Provide coverage in accordance with established staffing schedules.
Attend and successfully complete all necessary training programs.
Perform other duties as assigned.
