Licensed Practical Nurse (Northern)
About The Position
This role involves providing skilled nursing care under the supervision of a Registered Nurse (RN). The Hospice Licensed Practical Nurse (LPN) will assist RNs in executing the plan of care, including performing specialized procedures as directed by the RN and physician's orders, in accordance with the state nurse practice act. The position requires active participation in interdisciplinary care planning to ensure the patient's plan of care remains current and any identified issues are addressed. The LPN will also support patient and family education regarding end-of-life care and maintain open communication with the RN Case Manager regarding any observed changes. A commitment to personal and vocational growth through professional development activities is expected.
Requirements
- Licensed Practical Nursing Degree
- CPR/BLS Certified
- A minimum of 1 year of Clinical experience
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation

Responsibilities
- Provide skilled nursing care under the supervision of an RN
- Assist Registered nurses in carrying out the plan of care.
- Assist RN in performing specialized procedures (as defined in the state nurse practice act) as directed by the RN under physician's orders.
- Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
- Assist reinforcement of patient and family education regarding end-of-life care.
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
- Keep the RNCM informed of changes noted during the process.
- Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
