At-Heart Home Care is seeking a passionate, dedicated Home Health LPN to join our team in Virginia Beach, VA. Our Home Health LPNs provide expert, patient-centered care. The Home Health LPN will provide 1:1 care for clients following the client's plan of care at all times while maintaining client's dignity and independence. Responsibilities include administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results. Accurate documentation of nursing actions, communication with family, case manager, and physicians, and documenting progress and outcomes for established goals are essential. The nurse will also inform the physician, case manager, and families of changes in the client's medical condition and needs, and coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand. The role ensures all patient needs are being met in accordance with the instructions of the physician.
Requirements
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver’s license, acceptable driving record and proof of car insurance
- CPR professional certification
Nice To Haves
- 1+ years of nursing experience preferred
- Home Health care experience preferred
Responsibilities
