About The Position

Mass General Brigham Home Care is seeking a Licensed Practical Nurse (LPN) to join their Minuteman team. This role focuses on providing 1:1 skilled nursing care directly to patients in their homes, supporting their recovery, independence, and management of chronic illnesses. The position emphasizes autonomy, meaningful patient relationships, and practicing at the top of one's license. Unlike hospital settings, this role avoids "management" of patients and instead concentrates on high-quality, hands-on clinical care and education to prevent hospital readmissions. The primary coverage area is Dorchester, with additional territories including Mission Hill, Roxbury, and Jamaica Plain.

Requirements

LPN professional license in Massachusetts
Minimum 1-year previous nursing in acute care or sub-acute care setting required
Effective written, verbal and electronic communications skills
Ability to work with various computer software required
Ability to communicate effectively in writing, verbally and electronically
Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population
Strong interpersonal skills with ability to work with varied population
Ability to work as a member of an interdisciplinary team
Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
Travel required in local geographic area

Nice To Haves

Home care experience preferred
knowledge of hand-held patient care computer devices preferred

Responsibilities

Deliver 1:1 skilled nursing care
Work collaboratively with RN Case Managers
Practice at the top of your license
Provide complex clinical treatments
Support patients recovering from surgery, illness, or chronic disease exacerbations
Provide quality nursing care to patients in various settings in the community.
Provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures and standards of practice.
