About The Position

Cypress Grove Rehab Center is seeking a Licensed Practical Nurse (LPN) for night shifts, with an every-other-weekend rotation and no on-call responsibilities. The nurse will build meaningful relationships with residents and staff by learning about residents’ life stories and using nursing skills to promote their physical and psychosocial well-being. The role calls for a supportive presence, teamwork, collaboration, leadership within the care team, and strong interpersonal communication to help provide excellent, coordinated care.

Requirements

• Current and valid Licensed Practical Nurse license in the state of Indiana.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in resident condition.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.
• Ability to work toward a shared goal of excellent resident care.
• Demonstrate Compassion, Accountability, Relationships, and Excellence.

Responsibilities

• Promote residents’ physical and psychosocial well-being through nursing care.
• Create a comforting and engaging atmosphere for residents.
• Promote teamwork within the care team to meet residents’ needs.
• Collaborate with nursing and other facility teams to provide coordinated, comprehensive care.
• Conduct thorough resident assessments and accurately document changes in condition.
• Manage medications.
