About The Position

As a Licensed Practical Nurse (LPN), you will support the health and well-being of individuals served by Oak Hill through a combination of clinical care, health monitoring, documentation, and direct support. This is a substitute/per diem position requiring a minimum of 15 hours per month, with a competitive hourly rate dependent on experience, plus a sign-on bonus.

Requirements

Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
Graduate of an accredited Practical Nursing program.
Valid driver's license with the ability to transport individuals in agency vehicles.
Current CPR and First Aid certification, or ability to obtain upon hire.
Strong clinical assessment, communication, and documentation skills.
Ability to pass required background checks, fingerprinting, and pre-employment health screenings.
Ability to complete and maintain required certifications, trainings, and regulatory requirements.

Nice To Haves

Experience supporting individuals with intellectual and developmental disabilities.
Experience in residential, community-based, long-term care, behavioral health, or human services settings.
Experience with medication administration, G-Tube care, and health record documentation.
Familiarity with DDS regulations and person-centered support practices.

Responsibilities

Providing Clinical and Nursing Care: Monitor, assess, and respond to health needs; follow physician orders; administer medications and treatments; monitor vital signs; support specialized medical needs (e.g., G-Tube care); respond to medical emergencies.
Coordinating Healthcare Services: Schedule and coordinate medical appointments; communicate with healthcare providers, pharmacies, guardians, and team members; maintain medication records and other documentation; monitor medication inventories and order supplies.
Promoting Health, Safety, and Wellness: Assess health concerns and make recommendations; observe and report changes in status; support preventive healthcare and wellness education; implement health-related policies and care plans.
Supporting Daily Living and Independence: Provide guidance with personal care activities; encourage independence; assist with meal preparation and dietary guidelines; support participation in activities.
Maintaining Documentation and Regulatory Compliance: Complete nursing notes, incident reports, and other records; participate in team meetings; ensure compliance with Oak Hill, DDS, DPH, HIPAA, and regulatory requirements; maintain confidentiality.
Supporting a Safe and Inclusive Environment: Apply First Aid, CPR, and emergency response procedures; utilize approved lifting and transfer techniques; assist with behavior support plans; safely transport individuals using agency vehicles.
Collaborating as a Team Member: Communicate effectively with colleagues, supervisors, families, and healthcare providers; participate in meetings and trainings; assist with orienting new employees; contribute to a positive team environment.
