About The Position

Grand Itasca Clinic & Hospital, a part of Fairview Health Services, is currently seeking an enthusiastic LPN/MA to join our team! Grand Itasca's LPN's/MA's work closely with providers to deliver quality patient care and service by maintaining patient flow, escorting patients through the clinic, identifying patient needs, collecting/verifying information, administering medications, updating electronic medical records, implementing methods as directed to improve care and the patient experience, maintaining exam/treatment rooms, ordering related supplies and display original thinking & creativity while working effectively as an important team member. POSITION: Licensed Practical Nurse (LPN) EMPLOYMENT TYPE: .5-.6 FTE (40-48 hours a pay period) WORK SCHEDULE: Days DEPARTMENT: Clinic Nursing, Cardiology Float Internal Posting Through: 5/20/2026

Requirements

Current LPN Minnesota state license, current Medical Assistant certification or registration required. Must maintain license/certification/registration.
Ability to work efficiently, accurately, and maintain positive working relationships.
BLS certification to be completed within two months of employment.
Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.
Maintains reliable and consistent attendance.

Nice To Haves

Experience in an ambulatory health care setting is preferred.

Responsibilities

Conducts a focused assessment though the collection and comparison of data to normal findings and the patient's current health status. Report of changes will be given to the appropriate licensed health care provider for on-going direction, patient management and delegation of care.
Participates with health care team in the development and modifications to the patient's health plan of care plan/orders.
Implements patient care interventions that are delegated, ordered or prescribed by a licensed health care provider.
Assigns nursing activities or tasks to other licensed practical nurses (LPN).
Assigns and monitors nursing task or activities that are delegated to certified medical assistants or any unlicensed personnel on the health care team.
Focus on patient care through the provision of safe /effective care, patient advocacy, assistance with patient evaluation to interventions and collaboration and communication with health care team members.
Provides health care information to patients and reviews education materials.
Provides input for policies and procedures
Accountable for the quality of care delivered, recognizes the limits of practice and competency, and preforms to the level of education knowledge and skill ordinarily expected of individuals who have competed an approved LPN education program.
