About The Position
We are seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care setting. This role is ideal for a compassionate and detail-oriented nurse committed to delivering patient-centered care and improving patient outcomes.
Requirements
- Minimum 2 years of recent LPN experience in long-term care, rehabilitation, or similar clinical setting
- Active New York LPN license
- BLS certification (American Heart Association required)
Responsibilities
- Plan, implement, and document patient-centered nursing care
- Perform patient assessments and develop individualized care plans
- Deliver nursing interventions based on best practices and clinical guidelines
- Monitor patient conditions and provide therapeutic care
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Support continuous improvement and evidence-based practices
- Ensure compliance with healthcare regulations and safety standards
- Perform additional duties as assigned
