About The Position

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Current LPN licensure in good standing with the Kentucky Board of Nursing.
  • Minimum of two (2) years of nursing experience.
  • Demonstrates compliance with accepted professional nursing standards and practices.
  • Understands and supports the philosophy of hospice care and the needs of terminally ill patients.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Nice To Haves

  • community health/hospice, medical-surgical, or long-term care experience preferred.

Responsibilities

  • Provides skilled and compassionate nursing care to hospice patients in their place of residence, including private homes, nursing homes, assisted living facilities, or other settings the patient calls home.
  • Under the direction and supervision of a Registered Nurse, delivers individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life.
  • Performs assigned nursing and personal care tasks.
  • Monitors and reports changes in patient condition.
  • Educates patients and caregivers.
  • Documents care accurately and timely within the electronic medical record (EMR).
