About The Position

Our comprehensive and integrated 24/7 programs are designed to offer the holistic care our clients need. With a staff of 950+ and an annual budget of $140 million, Project Renewal is one of the larger social service nonprofits in New York City. The Parole Support and Treatment Program will provide temporary, supported housing and clinical services to men/women that are under the supervision of the Department of Corrections & Community Supervision with serious and persistent mental illness and co-occurring substance abuse disorders. This team will be responsible for the direct delivery of a broad range of services geared towards individual clients’ stabilization and transition into long-term community-based housing.

Requirements

  • New York State licensed LPN
  • Working with homeless individuals preferred
  • Must have excellent oral and listening skills
  • Must have the ability to interface with clients as well as all levels of staff.
  • Must have knowledge of Microsoft Office, especially Word and Excel.
  • Familiarity with electronic medical records documentation; eCW knowledge a plus

Responsibilities

  • LPNs provide on-site nursing services under the direction of the Nurse Manager.
  • Participate in weekly clinical meetings with medical staff, shelter case management, & administrative staff
  • Management of medication systems, medication self-administration records (MSAR) for clients who self-administer their medications in shelter, including: Ensuring safe storage of medications, Assisting clients with pill-packing of their medications, Ensuring clients have appropriate supplies and access to medications, Assisting with medication refills and attaining new prescriptions, Monitoring client reactions for side effects and reporting to prescribing provider
  • Maintain records of controlled substances using controlled substance logs
  • Education on medication adherence
  • Fulfill patient specific orders designated by the medical provider
  • Obtain vital signs per protocol
  • Administer medications, injections, and treatments as ordered by medical providers
  • Provide nursing screenings and documentation in nursing templates
  • Perform ordered procedures including, but not limited to, EKG’s, Hearing and Vision tests, Blood Glucose, Blood and urine specimen collection and processing, etc.
  • Order stock medications, immunizations, and other medical supplies as needed
  • Maintain emergency equipment, AED, oxygen tanks, and other clinical equipment
  • Monitor medication expiration dates
  • Discard expired medication
  • Record refrigerator and freezer temperatures using designated datalogger
  • Report on excursions or warning alerts
  • Travel to cover locations as necessary for coverage
  • Other duties as assigned
