About The Position

Works under the supervision of the Registered Nurse. The LPN's primary job function is to assist the RN in assessing, planning, implementing, evaluating, and supervising individual patient care on a nursing unit according to unit policies and procedures. Assists the RN with implementation of plan of care. Supports each strategic value by providing excellent care to those we serve. Performs other duties as assigned.

Requirements

  • High School Diploma or successful completion of an equivalent High School Exam Required
  • Graduate of an accredited school of practical nursing required
  • Licensed to practice as an LPN in Ohio required
  • IV required
  • BLS within 90 days of hire required
  • Ability to develop positive interactions with patients, patient families, physicians/providers, and staff in order to effectively care for the patient.
  • Hand/eye coordination in order to give injections, start IVs, use computer, etc.
  • Speak clearly to communicate with patients, patient families, staff, and physicians/providers regarding patient condition and care.
  • The physical demand is heavy. Occasionally lifts in excess of 100 pounds and frequently carries 50 pounds.
  • Stands or walks for greater than 50% of the shift with regular breaks when assessing and interviewing patients and providing skilled nursing.
  • Sits for one to three hours per shift when reviewing diagnostic data and completing required documentation.
  • Ability to use hands for repetitive simple grasping, fine manipulation, and pushing and pulling when attending to a patient's physical needs, operating patient care equipment, and recording responses to medications and treatment.
  • Occasionally bends, squats, climbs, crawls, and reaches above shoulder level when rendering skilled nursing.
  • Has considerable contact with the general public and customer when performing all aspects of job.
  • Occasionally works under close supervision and without the assistance of other personnel.
  • Requires the ability to cope with deadlines, adjust to irregular activity schedules, concentrates, and pay close attention to detail when performing patient assessments, completing required documentation and preparing and administering medication.
  • Good vision is required when providing skilled nursing, reviewing diagnostic data, and completing required documentation.
  • The ability to communicate clearly is required when instructing and comforting patients and their families and maintaining effective contact with physicians/providers and a variety of hospital personnel.
  • Occasionally exposed to dust, odors, body fluids, toxic substances, unpleasant patient care activities, and infectious diseases. Frequently exposed to noise.

Responsibilities

  • Assists in completion of the nursing assessment of patients and documents appropriately on patient record.
  • Assists with developing, evaluating and updating a plan of care based upon the assessment of the patient.
  • Assists with the discharge planning process.
  • Provides, delegates, and coordinates patient care.
  • Facilitates patient/family teaching as appropriate.
  • Participates in hourly rounding and bedside shift report.
  • Answers call lights and responds to patient needs.
  • Performs total patient care.
  • Pre-cleans, decontaminates and transports contaminated instruments according to policy.
  • Performs intermediate-level disinfection and/or high level disinfection per the Instructions for Use (IFU) by the manufacture. This includes ensuring the scopes or probes used within the department are cleaned, disinfected, maintained and stored per the IFU’s.
  • Performs other duties as assigned.
