About The Position
Pinnacle Home Care is hiring a full-time Licensed Practical Nurse (LPN) to provide compassionate, patient-centered home health care. The nurse administers skilled nursing care under RN supervision and performs curative, rehabilitative, and preventive tasks outlined in patient care plans. The role involves making informed care decisions, documenting visits in the electronic medical record, coordinating schedules, and following agency policies and quality, complaint-management, patient-rights, and discharge processes. The work includes visiting patients in their homes, which may not be wheelchair accessible.
Requirements
- Current state-specific LPN or LVN license.
- Graduation from an accredited nursing program.
- Valid BLS certification.
- At least one year of nursing experience; home health or medical/surgical experience is preferred.
- Excellent communication, organizational, and time-management skills.
- Valid driver’s license and access to a reliable, insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Ability to walk, stand, stoop, and occasionally lift or turn patients.

Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
- Use clinical expertise and nursing education to make informed decisions that support patient care and positive outcomes.
- Complete visit reports and required documentation promptly and accurately in the electronic medical record system.
- Confirm daily visit schedules to coordinate with other staff.
- Understand and participate as required in the agency’s Quality Assurance and Performance Improvement (QAPI) program.
- Follow the agency’s process for managing and addressing customer complaints.
- Uphold patient rights in accordance with agency service agreements and admission policies.
- Understand and follow agency policies regarding patient discharge and reasons for discharge.
