About The Position

The Licensed Practical Nurse (LPN) provides nursing care and assists in the planning and implementation of resident care focused on comfort, wellness, and quality outcomes. Working under the supervision of a Registered Nurse or licensed practitioner, the LPN participates in assessments, delivers treatments, monitors resident conditions, and contributes to individualized care plans. Carlyle Place, part of Atrium Health Navicent, is a premier not-for-profit Life Plan Community located in northwest Macon, Georgia. As an LPN at Carlyle Place, you'll have the unique opportunity to care for residents across multiple levels of care, including: Skilled Nursing, Assisted Living, Memory Care. Our community includes 226 independent living residences, 29 assisted living residences, 26 memory care residences, and 40 skilled nursing residences, creating a diverse and rewarding clinical environment.

Requirements

  • Graduate of an accredited School of Practical Nursing.
  • Current, unrestricted Georgia LPN license (or compact license eligible for practice in Georgia).
  • Current CPR certification or ability to obtain within 60 days of hire.
  • Basic Life Support (BLS) certification required per policy.

Nice To Haves

  • Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

  • Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
  • Monitor resident conditions and report significant changes to the healthcare team.
  • Assist with assessments, care planning, treatments, and procedures.
  • Administer medications and treatments within scope of practice.
  • Educate residents and families regarding care plans and treatments.
  • Document assessments, interventions, and resident responses accurately and timely.
  • Collaborate with interdisciplinary team members to support positive resident outcomes.
  • Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements.
