About The Position

AccordCare Home Health is seeking a Licensed Practical Nurse (LPN) to provide patient-centered clinical care under the supervision of a Registered Nurse and in accordance with physician orders, agency policies, and regulatory standards. The role focuses on safe, goal-oriented care delivery, strong communication with patients and families, and active contribution to the organization’s clinical excellence and growth. Requirements

  • Current, active LPN license in the state of practice, in good standing.
  • Minimum of one year of clinical nursing experience preferred; home health experience is desirable.
  • Strong communication and documentation skills.
  • Ability to work effectively as part of a healthcare team.
  • Commitment to safe, compassionate, and patient-centered care.
  • This role requires a screening through the Care Provider Background Screening Clearinghouse.

Responsibilities

  • Deliver skilled nursing care and support the development and execution of individualized, goal-oriented care plans.
  • Perform ongoing patient assessments and promptly report changes in condition to the supervising RN or Clinical Manager.
  • Administer medications and treatments accurately, following established protocols and maintaining strict aseptic technique.
  • Provide patient and family education on treatments, health management, and self-care techniques.
  • Document care clearly and on time in accordance with agency and regulatory standards.
  • Participate in interdisciplinary team meetings and quality improvement activities.
  • Promote patient rights and adhere to HIPAA, infection control, and safety protocols.
  • Collaborate effectively with the healthcare team, orient new staff as needed, and model professional conduct.
  • Manage time effectively to meet clinical and administrative requirements while maintaining high standards of care.
