About The Position

This is a temporary, full-time Licensed Practical Nurse position for 12-36 weeks at Sacred Heart Village in the Skilled Nursing department. The role contributes to patient assessment and well-being, administers medications, performs laboratory tests and therapeutic services, and educates patients and families about their treatment plans, self-care, resources, and follow-up care.

Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date required.
• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Responsibilities

• Contributes to the assessment of patients and assures the well-being of patients.
• Administers medications and observes patients for adverse reactions to medications or treatments.
• Performs laboratory tests and therapeutic services.
• Educates patients/families about disease treatment plan including self care post discharge, holistic health needs, available resources and follow up care.
