The LPN provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants. This role involves keeping administrative nursing staff informed on personnel development, providing feedback to Nurse Managers regarding the performance of nursing assistants, and participating in committees as assigned. The position requires the ability to address service concerns and follow up in a timely manner for resolution, supporting the facility’s Service Excellence Initiative. The LPN effectively supervises and directs licensed and non-licensed nursing personnel, develops CNA resident care assignments, and promotes positive working relationships with physicians, other healthcare personnel, nursing staff, other departments, residents, families, and visitors. Key responsibilities include observing residents, reporting significant changes in condition to the physician, completing routine nursing documentation accurately and timely, and administering medications using the five rights of medication administration while adhering to infection prevention and control practices. The role also involves implementing pharmacy processes, performing physical assessments, wound care, assisting in the development and updating of the resident's plan of care, and communicating effectively with family members. The LPN completes accident/incident reports, notifies physicians and families of incidents, and performs various nursing skills such as phlebotomy, IV therapy, wound care, medication administration, catheter insertion, tube feeding management, oxygen administration, tracheostomy care, airway management, and physical/skin assessments. The LPN also communicates staffing needs and concerns to the unit manager, receives and provides end-of-shift reports, assists with resident admissions, transfers, and discharges, and reports maintenance and housekeeping needs. Accurate narcotic record maintenance and validation are also required.
