About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) in the Clearwater, FL area. This role offers a flexible schedule and the opportunity for autonomy, allowing you to create and manage your own daily schedule. You will provide meaningful one-on-one care in patients' homes, with a coverage area in Pinellas County, FL, designed to minimize drive time through clustered scheduling. The position emphasizes work-life balance, avoiding hospital shifts or 12-hour rotations.
Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification

Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse
- Perform patient assessments and assist with care planning
- Administer medications and treatments as ordered
- Educate patients and caregivers on disease management and care plans
- Monitor patient condition and report changes to the supervising RN
- Collaborate with physicians and interdisciplinary teams
- Complete documentation using streamlined, AI-assisted tools
