About The Position

The Licensed Practical Nurse (LPN) at Columbia Healthcare Center provides nursing care that promotes residents’ physical and psychosocial well-being. The role emphasizes building meaningful relationships with residents and staff by learning their life stories, creating a supportive atmosphere, and working collaboratively with the care team to deliver coordinated care. This is a full-time day-shift position with 12-hour shifts from 6 AM to 6 PM, a weekend rotation, and no on-call requirement.

Requirements

• Current and valid Licensed Practical Nurse license in the state of Indiana.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in resident condition.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.

Responsibilities

• Promote the physical and psychosocial well-being of residents.
