About The Position

AgeCare Glenwarren is seeking a Licensed Practical Nurse (LPN) to join their team. Reporting to the Director of Care, the LPN will play a key role in delivering goal-directed, patient-focused nursing care in accordance with provincial standards. As a member of an interdisciplinary team, the LPN will ensure residents receive high-quality, compassionate care that supports their dignity and well-being. The role involves working collaboratively with the care team, conducting resident assessments, implementing and evaluating care plans, providing direct nursing care, administering medications, and coordinating care with various stakeholders. The LPN will also be responsible for responding to changes in residents' conditions, managing medical emergencies, delegating tasks to unregulated care providers, and supporting infection prevention and quality improvement initiatives. The position requires acting with integrity and upholding the organization's values.

Requirements

  • Current registration as a Licensed Practical Nurse (LPN) in the applicable province/state.
  • Current CPR and First Aid certification.
  • Proficiency with electronic documentation and Microsoft Office.
  • Completion of a current Vulnerable Sector Check (or willingness to obtain).

Nice To Haves

  • Experience in senior care or long-term care (preferred).
  • Education or background in gerontology, rehabilitation, or complex/community care (asset).
  • Strong clinical knowledge and care coordination skills
  • Excellent communication, critical thinking, and time management abilities.

Responsibilities

  • Work collaboratively with the care team using a person-centered care approach.
  • Conduct and maintain accurate resident assessments, care plans, and documentation; communicate updates as needed.
  • Implement and evaluate care plans, adjusting as required.
  • Participate in resident care conferences and support scheduling.
  • Assist with resident transitions including move-ins, discharges, and transfers.
  • Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
  • Safely manage medications and narcotics in accordance with scope of practice and legislation.
  • Educate residents and families about health conditions and care plans.
  • Coordinate care plans with physicians, team members, and service providers.
  • Assign responsibilities to the nursing team for efficient service delivery.
  • Respond to significant changes in residents’ conditions and manage medical emergencies.
  • Provide direction and delegate tasks to unregulated care providers.
  • Oversee emergency response in the absence of senior leadership.
  • Address resident/family concerns and ensure timely follow-up or escalation.
  • Support infection prevention, health and safety initiatives, and quality improvement plans.
  • Act with integrity and uphold the organization's values, particularly those related to ethics and trust.
  • Perform other related duties as assigned.
