About The Position

This role is for a Licensed Practical Nurse (LPN) with Summa Home Health, focusing on providing compassionate quality care and positive outcomes for patients in a home-based setting. The LPN will be part of an interdisciplinary team, delivering nursing care as directed by the attending physician and following order sets and goals created by the Case Manager. The position involves interacting with patients' families and supporting patients through their recovery journey, offering a real-world perspective on health restoration. The LPN will work autonomously, managing their time efficiently, and will be responsible for all practices and duties within the scope of practice outlined by the state.

Requirements

  • Licensed Practical Nurse (LPN) with current license in the state of employment.
  • Minimum one-year experience as a LPN in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
  • Compassionate communicator with a positive attitude.
  • Patience when working with patients, families, physicians, and coworkers.
  • Attention to detail.
  • Observant.
  • Following directions.

Responsibilities

  • Provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager.
  • Interact with patients' families while caring for patients.
  • Provide skilled nursing care in a patient's home as directed by the attending physician.
  • Work autonomously and efficiently manage time.
  • Be responsible for all practices and duties within the scope of practice outlined by the state.
  • Provide skilled nursing care as ordered by the attending physician.
  • Observe and monitor patient conditions and notify case manager of any changes in the patient’s condition.
  • Administer medication as prescribed by the physician.
  • Help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
  • Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
