About The Position

S:US is seeking an LPN for their adult family shelter in Manhattan. The role involves the diagnosis and management of acute, chronic, and complex health problems, including assessing, diagnosing, and treating illnesses, and educating shelter clients and staff on disease prevention. The LPN will provide health education and counseling to individuals residing in the shelter.

Requirements

  • Completion of accredited program and licensed in New York as an LPN.

Responsibilities

  • Meets with Program Director, Director of Social Services, Psychiatrist, Program Nurses, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.
  • Supervise Medication Coordinator and assist Psychiatrist and Psychiatric Nurse Practitioner as needed.
  • Administer medications and document administration as required.
  • Take vital signs, such as blood pressure, temperature and weight.
  • Assist with diabetes medication and treatment.
  • Take medical histories and enter information in computer systems.
  • Coordinate with Case Manager, Senior Case Manager and Director of Social Services to ensure proper care, nutrition and treatment plans are followed.
  • Follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed.
  • Monitor MARs and medication storage.
  • Work with residents and Case Managers in filling prescriptions and ensuring refills are ordered as necessary.
  • Ensure consumer safety, assess consumer reactions to medications, assist with consumer nutritional needs.
  • Ensure physician orders are current, properly documented and communicated to staff.
  • Monitor client medical equipment for safety and proper functioning.
  • Attend and participate in regular staff training and scheduling meetings.
  • Maintain confidentiality and privacy regarding all clients’ information.
  • Complete daily documentation of all agency required material.
  • Complete incident reports in accordance with policy and procedures.
  • Understand and implement the agency’s philosophy, policies and mission.
  • Understand and follow the agency chain of communication.
  • Ensure the safety and well‐being of the clients on a daily basis.
  • Work cooperatively with other team members to carry out the program goals and objectives for each client.
  • Report any actual or suspected abuse/neglect in accordance with policy and procedure.
  • Prepare clients for self‐medication status and monitor as needed, health assessment referrals.
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
  • Maintain CPR/First Aid Certification.
  • Assist in building evacuation during fire drills and building emergencies.
  • Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
