Participates in the planning, implementation, and evaluation of nursing care for individuals with developmental disabilities and/or mental health diagnoses. Cooperates with members of the nursing and medical staffs. This role involves assisting in relevant health teaching for residents and staff, sharing responsibility for evaluating health needs, and participating in medication distribution and team conferences. The LPN will administer and record medications, perform complex nursing activities as competence increases, and participate in emergency care. They will communicate frequently with the RN regarding patients' conditions, record information concisely and accurately, and schedule/track medical and therapy appointments. This includes maintaining medical and therapy grids, ensuring individuals served and team members are informed of appointments, and noting all appointments and follow-up in electronic or hard copy records. The LPN will also receive and upload consultation forms to Therap and perform other assigned duties.
