About The Position

The FACT Licensed Practical Nurse (LPN) conducts psychiatric assessments, assesses physical health needs, coordinates referrals to community health care providers, and ensures proper medication self-administration in conjunction with the Psychiatrist or Psychiatric Advanced Practice Registered Nurse (APRN). The LPN serves as primary case manager for an assigned group of individuals and promotes recovery, empowerment, self-determination and decision-making for each FACT participant. The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life. FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers. Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members must also use their personal vehicles as needed and are eligible for mileage reimbursement. The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities.

Requirements

  • Graduation from an accredited school of Practical Nursing required.
  • Valid Florida Licensed Practical Nurse (LPN) license in good standing required.
  • Six months of medical/surgical and/or psychiatric nursing experience required.
  • Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
  • Proficiency in Microsoft Office, Outlook and the use of the Internet.
  • Must meet MHRC/RBHS driver requirements, including a valid Florida driver’s license, required insurance coverage, and acceptable driving record.
  • Ability to work in a variety of settings, including homes, offices, hospitals, nursing homes, and secure units.
  • Ability to make sound, independent decisions, including responding to emergencies and high-risk situations.
  • Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers, and team members.

Responsibilities

  • Provides treatment, rehabilitation, and support services in accordance with the FACT model (based on the PACT model).
  • Maintains accurate, complete, and confidential clinical records.
  • Conducts mental health status assessments.
  • Coordinates referrals to community providers and support services.
  • Coordinates referrals to community health care providers
  • Provides education on mental health symptoms, medications, and recovery.
  • Ensures proper medication self-administration in conjunction with the ARNP or Psychiatrist
  • Serves as primary case manager for an assigned caseload, promoting recovery, empowerment, and self-determination.

  • Transforms brief notes or session conversations into complete, clinically appropriate documentation
