About The Position

Join our team providing private duty home care for a patient in the Grove City, OH 43123 area. This is an excellent opportunity for an LPN with tracheostomy (trach) and G-tube experience who enjoys building meaningful relationships while delivering high-quality, individualized care in a home setting.

Requirements

• Current Ohio Licensed Practical Nurse (LPN) license in good standing
• Experience with tracheostomy (trach) care required
• Experience with G-tube care and enteral feedings required
• Current CPR certification
• Excellent assessment, communication, and documentation skills
• Dependable, compassionate, and committed to providing exceptional patient care

Responsibilities

• Provide skilled one-on-one nursing care in the patient's home
• Perform tracheostomy (trach) care and suctioning as needed
• Manage G-tube feedings and care
• Administer medications as ordered
• Monitor and document patient condition and nursing interventions
• Communicate effectively with the patient's family and healthcare team
• Ensure a safe, comfortable, and supportive care environment
