About The Position

In a team relationship with the Registered Nurse, the Licensed Practical Nurse contributes to the nursing process through on-going systematic data collection, assisting with the health assessment, recording observations, goal identification and patient health education. The Licensed Practical Nurse will also collaborate with the healthcare team, ensure health and safety compliance, and perform clinical tasks. Performance Expectations include contributing to nursing assessment, care, and planning in conjunction with the RN supervisor. The role involves initiating the admission process, assisting with the identification of recurrent health problems, symptoms, and behavioral changes. Data collection through observation and interview, and communication of patient condition changes are key. The LPN will record assessment data, assist in forming a written plan, and identify short and long-term goals with anticipated behavioral changes. They will assist in setting priorities for patient care, assist with patient healthcare, and participate in multi-disciplinary care conferences. Additional duties include counting CDS Drugs and contraband if required, assisting physicians in minor medical/surgical procedures, transcribing and posting therapeutic orders, and initiating and documenting prescribed treatments. The LPN must practice universal precautions, maintain a neat, clean, and safe environment, keep patient information confidential, and respect patient privacy. They must keep the health team informed of observations and changing patient conditions. Maintaining active state nursing licensure with bi-annual proof is required. Adherence to dress code (white lab coat) and prompt arrival for verbal report are expected. A doctor's excuse may be required for absences before or after scheduled days off. Notification of illness during duty and disposition is necessary. Requests for vacation or time-off must be submitted in advance, and workday exchanges require written notification to the Charge Nurse and/or RN Supervisor.

Requirements

  • Graduate of an accredited Licensed Practical Nurse program with current licensure in the state of employment
  • Must conform to the security clearance standards of the institution were employed
  • Ability to pass all required health and other screening tests
  • Must maintain and show bi-annual proof of active state nursing licensure with a copy to be placed in the personnel file maintained by the RN Supervisor
  • Punctual and dependent for assigned/confirmed shifts

  • Will notify the RN Supervisor if becoming ill on duty to notify him/her of the disposition
  • Submits request for vacation or special time-off in advance of the posting of scheduled time
  • Arrange workday exchange in the same pay period with a comparable member of the nursing staff for the desired day off if the schedule has already been posted
  • After the agreement is reached, the Charge Nurse and/or the RN Supervisor are to be notified in writing by both parties of the desired schedule change agreement

  • Participate in ongoing quality improvement activities
  • Completes required orientation as directed by facility
