Requirements

Current Massachusetts Licensed Practical Nurse (LPN) license
Minimum of one (1) year of clinical nursing experience
Afternoon availability

Previous home care experience
Monday–Friday availability

Responsibilities

Provide skilled nursing care in patients' homes, including wound care, medication management, catheter care, diabetic care and education, wellness assessments, disease management education, and follow-up after physician visits.
Order medical supplies as needed.
Accurate, timely electronic documentation.
