About The Position

The Licensed Practical Nurse (LPN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Requirements

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice
• One year of experience under license
• 1 year Hospice/Home Health experience
• Cn-call and Triage experience
• Ability to work well within a team environment to collaborate daily with a team of clinicians
• Ability to work independently and on your own from a day-to-day basis
• Understanding of state and federal rules and regulations
• Time management skills to manage visits, documentation, meetings
• Ability to learn an electronic medical records system
• Willingness to drive and reliable transportation
• Valid driver’s license
• Auto insurance

Nice To Haves

• Ability to work closely with patients on a 1-1 level but get strong support from a patient care team
• Activities which include: phone, email and written communication, meetings with a variety of healthcare professionals
