About The Position

The Connecticut Valley Hospital (CVH) Blue Hills Campus, under the umbrella of the State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking a Licensed Practical Nurse (PCN 25594) to join our team in Hartford. This position will support the admission and screening of patients entering substance use disorder treatment.

Requirements

  • License or temporary permit to practice nursing in Connecticut by the State Board of Examiners for Nursing OR hold a Multistate or Compact Licensed Practical Nurse license.

Nice To Haves

  • Experience working in Emergency Department, Withdrawal management units or Behavioral Health inpatient programs treating patients with co-occurring disorders.
  • Experience using interpersonal skills to support, calm and deescalate clients/patients.
  • Experience communicating and collaborating with external referral sources and multidisciplinary clinical teams.

Responsibilities

  • Collaborate with RNs, providers, and the interdisciplinary team to ensure safe, timely, and efficient patient admissions.
  • Conduct clinical screenings for individuals seeking admission to substance use disorder (SUD) treatment services.
  • Obtain and document vital signs, nursing history, allergies, medications, and other relevant clinical information.
  • Monitor patients for withdrawal symptoms, condition changes, and other clinical concerns, promptly reporting to the RN or provider.
  • Assist with medication reconciliation, specimen collection, point-of-care testing, and other nursing procedures within LPN scope of practice.
  • Administer medications and treatments in alignment with provider orders, organizational policies, and the LPN scope of practice.
  • Maintain accurate, timely, and comprehensive clinical documentation.
  • Support trauma-informed, recovery-oriented, and patient-centered care practices.
