About The Position

Midland Care Connection’s Palliative Care program is dedicated to providing compassionate, holistic care to individuals living with serious or life-limiting illnesses. Our team focuses on comfort, dignity, symptom management, and quality of life while supporting patients and their families through every step of the journey. Through an interdisciplinary approach, Midland Care provides personalized medical, emotional, spiritual, and practical support in the setting that best meets each patient’s needs. As a Palliative Care LPN, you'll provide compassionate nursing care to seriously ill patients and their families while working closely with Midland Care’s interdisciplinary Palliative Care team. This role is primarily office-based and includes setting up and maintaining patient charts, coordinating care activities, managing telephone communications, and conducting patient follow-up calls to monitor needs and support continuity of care. You'll connect with patients through telephone and virtual visits, in the clinic, and occasionally in their homes, helping assess and address care needs, support medication management, provide education, and coordinate services. While the position focuses largely on care coordination and clinical support, it also includes some direct patient care as needed. You'll collaborate with nurses, providers, pharmacies, community agencies, and other healthcare professionals to ensure patients' needs, goals, and care plans are effectively addressed. This role also involves maintaining accurate documentation, participating in care planning and quality improvement activities, and building meaningful, therapeutic relationships with patients and families throughout their care journey.
