Your clinical skills and compassionate approach will help ensure each elder maintains the highest level of independence, health, and well-being.

Requirements

  • Active LPN License in Pennsylvania
  • Graduate of an Accredited Nursing Program
  • Temporary Practice Permit (TPP): If you are a new graduate, you hold a valid TPP issued by the PA State Board of Nursing.
  • CPR Certified
  • Safety-First Mindset: You follow all infection control and universal precaution protocols and understand the importance of PPE in protecting yourself and others.

Responsibilities

  • Monitor and Respond to Changes in Condition: Proactively assess residents, identify early signs of decline, and communicate changes to the appropriate team members and physicians.
  • Deliver Safe, Effective Treatments: Administer medications and treatments as prescribed, track and document outcomes in the EHR, and report adverse reactions promptly.
  • Develop and Maintain Individualized Care Plans: Complete initial assessments, contribute to care planning, attend interdisciplinary meetings, and update plans as conditions change.
  • Lead in Emergency Situations: Respond swiftly to medical emergencies, follow elder-specific directives, and coordinate care transitions with families, hospitals, and ambulance services.
  • Foster Open Communication: Collaborate with elders, families, and the care team to ensure needs are met and concerns are addressed. Make referrals to other departments as needed.
  • Maintain Accurate Documentation: Keep thorough, timely records in both electronic and paper formats, ensuring compliance with facility protocols and legal standards.
  • Promote Resident Safety and Well-Being: Complete incident reports, implement preventive interventions, and ensure each elder is supported in participating in activities and services.
  • Support Team Communication and Continuity of Care: Give and receive detailed shift reports, brief nursing assistants, and maintain smooth handoffs between shifts.
  • Handle Medications and Inventory with Precision: Manage controlled substances, perform shift-to-shift counts, and report discrepancies immediately in accordance with policy.
  • Implement Physician Orders: Transcribe, clarify, and carry out orders from physicians accurately and efficiently.
  • Mentor and Support New Team Members: Help orient and guide new staff, sharing best practices and promoting a culture of collaboration and learning.
  • Stay Sharp and Compliant: Participate in ongoing training, fulfill all continuing education requirements, and ensure strict adherence to infection control protocols.
