About The Position
Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a full-time opportunity on our day shift for Racine - Belle City, 5200 Washington Ave., Racine, WI 53406.
Requirements
- Diploma from accredited program in practical nursing required.
- A current Wisconsin Licensed Practical Nurse/LPN license in good standing
- Current CPR/AED certification
Nice To Haves
- primary work is in clinic but will need to make home visits also
Responsibilities
- Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operations.
- Serves as coordinator of care for scheduled weekend and holiday shifts.
- Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.
