About The Position

Stamford Health is seeking a part-time Licensed Practical Nurse (LPN) for its Urogyn & Recon Surg practice. The LPN will coordinate patient care in accordance with established standards and guidelines, provide staff mentorship, and serve as a resource for problem identification and resolution. The role involves rotating between Norwalk and Stamford locations.

Requirements

  • Graduate of an accredited school of nursing.
  • Valid license as Licensed Practical Nurse in CT.
  • BLS Certification
  • Excellent customer service / communication skills.
  • Knowledge of medical terminology.
  • Must be able to read, write and speak the English language.

Nice To Haves

  • Minimum of 2 years previous experience in a similar position preferred

Responsibilities

  • Reviews schedules and organizes patient flow; greets patients, documents concerns and questions, accompanies patients to exam/procedure rooms, and prepares them for physical exams.
  • Conducts initial patient interviews, reviews medical profiles, and documents all required charting in EMR or manual charts.
  • Obtains and accurately documents patient vital signs and weight, reporting any abnormalities to the physician.
  • Assists with specimen collection and testing as necessary, documenting pertinent information.
  • Assists patients with mobility, transfers, and dressing as needed.
  • Provides minor treatments as directed by the physician and assists with minor procedures.
  • Educates patients and families based on their learning needs.
  • Schedules patient testing and obtains pre-certification if required by insurance.
  • Responds to telephone messages from patients and physician offices.
  • Conducts end-of-month checks for expired supplies and medications.
  • Stocks rooms and maintains inventory of exam/procedure rooms.
  • Orders supplies as needed.
  • Ensures exam/procedure rooms and equipment are cleaned after each patient.
  • Prepares exam/procedure rooms for the next patient and physician needs.
  • Notifies patients of test results and follows up as instructed by the physician.
  • Attends scheduled staff meetings and in-service education opportunities.
  • Participates in continuing education programs.
  • Maintains effective working relationships with employees and other members of the healthcare team.
  • Respects and protects patient confidentiality and privacy, disclosing information only for professional purposes in the patient's best interest and with consideration of their legal rights.
  • Displays a positive attitude when interacting with physicians' office staff, physicians, and fellow employees.
