About The Position

The Licensed Practical Nurse (LPN) will provide comprehensive nursing services for all assigned participants. This role involves conducting wellness assessments, developing and overseeing coordination plans, training staff, consulting with healthcare providers, and providing direct nursing care. The LPN will also be responsible for medication administration, maintaining health records, and ensuring compliance with state regulations. This position requires flexibility, strong communication skills, and the ability to work both independently and as part of a team.

  • Licensed Practical Nurse with a current Indiana license is required.
  • At least one-year experience working with individuals with developmental disabilities is required.
  • Current LPN License.
  • Medication Administration: Core A and Core B.
  • CPR/First Aid certification.

Responsibilities

  • Complete State’s Wellness Assessment for assigned participants at state-required intervals, including demographic information, vital signs, health history, physician's orders, risk assessments, and examinations.
  • Develop, oversee, and maintain assigned participants' Wellness Coordination plan.
  • Train Direct Support Professionals on physician orders, wellness needs, and provide updated information on participant wellness/health needs.
  • Consult with each participant’s healthcare providers as needed.
  • Provide weekly consultations and reviews for all assigned participants, with face-to-face visits as appropriate for the Wellness Tier.
  • Communicate wellness/risk changes to the RN that require updating in participants' Wellness/Risk Plan.
  • Visit SFC participants monthly at the Family Caregiving site, record vital signs, and review all medications and MAR.
  • Advise employees on proper nursing care and potential adverse reactions, interactions, and allergies of prescribed medications (as assigned).
  • Complete a quarterly examination for any assigned participant, make referrals to a physician as needed (as assigned), and complete a quarterly health assessment.
  • Train assigned group home participants and staff on caring for health needs/conditions, personal hygiene, health maintenance, and disease prevention.
  • Promote health and wellness and take steps to address problems before they become serious.
  • Review all Medical Information Forms (MIFs) for changes in physicians’ orders and ensure all changes are implemented.
  • Assist in maintaining the EMAR.
  • Provide health care coordination to participants without Wellness Coordination and visit to evaluate each group home individual’s physical status as needed.
  • Provide direct nursing care when appropriate.
  • Teach Core A, Core B, and other Health Services classes (as assigned).
  • Assist with lifting, turning, moving, positioning, and transporting participants into and out of beds, chairs, wheelchairs, lifts, etc. (as needed).
  • Carry a designated phone to provide continuous after-hours phone support to direct care employees on a rotating basis.
  • Act as a backup for the Pharmacy Specialist.
  • Maintain, review, and update all dietary documents.
  • Work under the direction of the RN or consulting physician for clinical/medical matters.
  • Administer medications as directed.
  • Model and maintain professionalism, confidentiality, and teamwork at all times, including when dealing with individuals from outside agencies.
  • Provide and maintain open lines of communication and good relationships with all employees at assigned facilities and administrative support employees through phone, e-mail, memos, or other professional means.
  • Perform duties as required to meet state regulations in the discharge of duties.
  • Perform other duties as deemed necessary by management.
  • Work additional hours if needed, which are mandatory.
