About The Position

The Terraces of Phoenix, a premier 5-Star HumanGood life plan community, is seeking a compassionate PRN Licensed Practical Nurse (LPN/LVN) for our Skilled Nursing Team. This role is responsible for the nursing needs of residents, while actively incorporating the "Philosophy for Person-Directed Care." The LPN will ensure a safe and healthy environment for residents, staff, and visitors, participate in the resident’s daily life rhythm, develop assignments in coordination with care partners, and keep each team member accountable for resident care and satisfaction.

Requirements

• Current LPN license in the state of Arizona
• Current CPR Certification
• Excellent assessment, documentation, communication, and organizational skills.
• Strong customer service and teamwork acumen.

Nice To Haves

• Experience in skilled nursing, subacute care, admissions, or case management is highly desirable.
• Current or prior experience with PCC is preferred.
