About The Position

Provide nursing care within the scope of licensure for assigned individuals according to established nursing standards and in accordance to DIDD and DOH Policy and Procedures with utmost professionalism. This role involves providing healthcare supports as prescribed by treating physicians, interpreting and implementing Individualized Health Care Plans, and assisting in the ADL process to ensure the safety and integrity of the individual. The position also includes administering medications, interpreting and implementing Medical Crisis Plans, ensuring medical equipment is maintained, and maintaining an accurate inventory of healthcare supplies. Documentation of care, education provided, orders followed, and medical interventions is crucial. The role also involves assisting with training new staff, attending ISP and COS meetings, completing Physical Status Reviews, and maintaining a current Health Passport. Reviewing therapy evaluations and clinical monthly reviews, accompanying individuals to doctor appointments, and maintaining consumer records in accordance with the DIDD Operations Manual are also key responsibilities. Specific duties related to Medication Administration Records (MARs), Medication Variances, Referrals/Follow-Up, and Therapy Services are detailed, along with responsibilities concerning Psychotropic Medications and Health Care Oversight, including seizure tracking, BM tracking, intake records, and weight monitoring.

  • Must possess a current license, in good standing, as a practical nurse issued from the State of Tennessee, or issuing state must be part of the Nursing Compact acceptable to the State of Tennessee.

Responsibilities

  • Provide healthcare supports as prescribed by treating physicians.
  • Interpret and Implement Individualized Health Care Plans (e.g., bowel and bladder tracking, monitoring and documenting input and output, seizure tracking, blood pressure monitoring, urostomy care, tracheotomy care, routine injections).
  • Assist in the ADL process (e.g., bathing, dressing, oral hygiene, toileting, feeding).
  • Administer medications, identify and report medication effectiveness and adverse side effects, and maintain medication counts for controlled drugs.
  • Interpret and Implement Medical Crisis Plans and report the need for revisions to the Agency RN.
  • Ensure that medical equipment is kept clean and serviceable.
  • Maintain an accurate inventory of healthcare supplies and report discrepancies to the Agency RN.
  • Document care and education provided to the individual served.
  • Document orders followed and medical interventions in the form of a detailed nursing summary.
  • Assist with shadowing, training, mentoring, and reviewing plans with new staff.
  • Attend and actively participate in Annual ISP and COS Meetings.
  • Complete the Physical Status Review (PSR) and Healthcare Oversight Form 90 days prior to the ISP effective date.
  • Maintain a current and accurate Health Passport, updating at least quarterly.
  • Review Therapy Evaluations and Clinical Monthly Reviews to ensure appropriate implementation of therapy plans.
  • Maintain current competency-based training for all therapy plans.
  • Accompany individual to all doctor appointments and keep Nursing Department abreast of any changes to the health care plan.
  • Maintain consumer records in accordance with DIDD Operations Manual.
  • Ensure that required paperwork such as Medical Encounter Forms, Medication Logs, and Medication Variances are submitted to the Nursing Department in a timely fashion.
  • Review MAR’s prior to implementation and initial every medication on the MAR as verification.
  • Ensure MAR’s adequately reflect all critical elements as outlined by DIDD.
  • Obtain specific parameters for all PRN medications prior to implementation.
  • Verify that all new orders are transcribed in accordance with the Medical Encounter Form AND the script, addressing any discrepancies.
  • Ensure that MAR Travel Books include accurate and complete MAR, corresponding scripts, Medication Profiles, and Medication Count Sheets.
  • Detect, document, and report medication variances immediately, including evidence of PCP/NP notification.
  • Ensure all Medication Variances are on file by the 5th of the month for the previous month.
  • Review all Medical Encounter Forms upon submission to ensure timely and adequate follow-up to referrals.
  • Verify all MD referrals to confirm timely scheduling and notify Agency RN if not scheduled.
  • Ensure that all ordered procedures are scheduled according to the order and notify Agency RN if not scheduled.
  • Request and obtain test and lab results within 1 week of the procedure.
  • Document all medical appointments on Medical Calendars and notify Agency RN if an appointment warrants rescheduling.
  • Ensure that individuals served are not seeing 2 physicians with the same specialty, unless one is clearly documented as a second opinion.
  • Stay abreast of Therapy Plans that would place the individual at risk of harm if not implemented (e.g., Dining Plan, Gait Plan, Bathing Plan).
  • Inspect assistive devices to ensure equipment is safe and sanitary.
  • Review necessary documents prior to psychiatric appointments to avoid unwarranted medication adjustments.
  • Ensure that all psychotropic medications have a current and valid consent.
  • Ensure that the Psychotropic Consent clearly captures strength and route for every psychotropic medication indicated.
  • Ensure that the Psychotropic Consent is completed by the Psychiatrist or his/her Nurse Practitioner.
  • Obtain verbal consent from the individual’s legal representative PRIOR TO administration of psychotropic medications, and obtain written consent within 14 working days.
  • Obtain written consent by the individual immediately if they do not have a legal representative.
  • Complete and Submit HRC Medication Review Form to the Agency RN.
  • NOT administer PRN psychotropic medications without prior approval by Agency RN or a written and approved Medical Crisis Plan.
  • Ensure that any individual with a history of or current seizure diagnosis has a seizure record in their chart.
  • Notify the Agency RN if an individual experiences an increase in seizure activity or experiences seizure activity after an extended lapse.
  • Ensure that a Daily BM Tracking Form is maintained and adequately recorded if an individual is on a stool softener for constipation.
  • Ensure that Daily Intake Records are maintained and entries are consistent to the order if an individual is on a physician-prescribed diet.
  • Review the Monthly RN Oversight Form prior to placement in the individuals Medical Chart and keep the Agency RN abreast of resolutions.
  • Demonstrate the ability to interpret and implement Medical Crisis Plans for individuals on his/her caseload (e.g., CHF, HTN, Diabetes).
  • Report a monthly weight loss or weight gain of 10 pounds or more to the Agency RN.
  • Notify the Agency RN if individual served experiences an increase in falls and implement a Fall Tracking Form.
  • Report substandard performance by another LPN to the Agency RN.
