About The Position

DaVita is seeking a Licensed Practical Nurse (LPN) who is looking to give life in a skilled nursing setting. You can make an exceptional difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease. DaVita Kidney Care is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This role involves providing dialysis care in skilled nursing facilities, aiming to reduce patient burden and healthcare costs. Teammates will have access to DaVita's resources and will work to develop and grow this new service line. The ideal candidate will have experience within DaVita, thrive in a start-up environment, be motivated by improving patient lives, and demonstrate a track record of getting things done. Comfort working autonomously and collaborating with cross-functional resources is essential. In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. LPNs will work in conjunction with dialysis RNs to provide dialysis care. This position involves direct patient care, managing complex cases, and utilizing critical thinking skills in an independent setting. You will monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup. Training will include classroom and hands-on learning through DaVita’s award-winning programs, potentially in a facility or training clinic. You may also be asked to support other local SNF dens. You will build meaningful relationships with patients and families in an intimate outpatient setting, be part of a supportive team, and deliver care to patients with multiple co-morbidities, requiring unique treatment plans and a broad range of nursing skills. You will work with your head, heart, and hands each day in a fast-paced environment.

Requirements

  • Current LPN-LVN license in the state of practice required
  • Current CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency in computer systems of DaVita within 90 days of hire required

Nice To Haves

  • Minimum of one year's experience in a hospital or clinical setting preferred

Responsibilities

  • Provide safe, comfortable, and hygienic dialysis treatment.
  • Work in conjunction with dialysis RNs to provide dialysis care.
  • Spend the majority of your day in direct one-on-one patient care.
  • Meet and/or exceed clinical outcomes.
  • Multitask to meet the needs of the service, patients, and SNF partners.
  • Provide comprehensive patient care in an independent setting.
  • Monitor patients before, during, and after dialysis treatment.
  • Measure and record stats, patient observations, and hemodialysis machine setup.
  • Build meaningful and long term relationships with patients and their family in an intimate outpatient setting.
  • Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.
