Community Access, founded in 1974, is a pioneer in supportive housing and social services for individuals with mental health concerns in NYC. We advocate for human rights, social justice, and economic opportunities. Our programs span 25 locations with over 1200 units of supportive housing across three boroughs, including the Howie the Harp Advocacy Center, a Crisis Respite Center, Assertive Community Treatment (ACT), and the Intensive Mobile Team (IMT). The Assertive Community Treatment (ACT) program is an evidence-based, mobile practice model providing treatment, rehabilitation, and support services to individuals with serious mental health conditions whose needs are not met by traditional services. The Bronx Shelter ACT Team specifically serves individuals with mental health conditions experiencing homelessness in the Bronx and other boroughs. Team members collaborate with shelter staff, using assertive engagement strategies to proactively involve individuals in services. They share responsibility for participants' well-being, assisting them in achieving goals related to housing, health, social life, work, and education. ACT teams are multidisciplinary, comprising psychiatry, nursing, social work, and peer specialists. They offer expertise in housing, substance use and harm reduction, family support, employment, and wellness self-management. Services are comprehensive, integrated, and flexible, delivered by a small staff-to-participant ratio in natural settings, 24/7. The Licensed Practical Nurse (LPN) is a primary practitioner who assesses and monitors ACT participants' clinical health and response to treatment. Utilizing the nursing process (assessment, planning, intervention, implementation, evaluation), the LPN assists individuals in achieving their mental health, medical, rehabilitation, and recovery goals.
