About The Position

The Maples is seeking Licensed Practical Nurses (LPNs) for full-time and PRN shifts to support The Maples Clinic in Hammond, LA. The role involves providing basic patient care under the supervision of a registered nurse, including taking vital signs, administering medications, preparing patients for exams, collecting specimens, assisting with procedures, educating patients on self-care, and documenting patient information. The LPN will maintain clear communication with the healthcare team and patients, participate in the planning, coordinating, and documenting of total patient care within a multidisciplinary treatment team, and ensure accurate and timely clinical documentation. Responsibilities also include monitoring and reporting client conditions, skin assessments, managing medication administration and storage, and ensuring all physician orders are followed.

Requirements

Graduate of an accredited school of nursing
Must have active Licensed Practical Nurse in the state of Louisiana.
Minimum of 1 year experience as an LPN
Certification in Basic Life Support
Certification in non-violent crisis intervention

Nice To Haves

experience in behavioral health care preferred

Responsibilities

Participates in the planning, coordinating, and documenting of total patient care in conjunction with the multidisciplinary treatment team
Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change
Accurately monitor skin assessments with proper documentation and reporting of changes
Completes, maintains, and documents accurate and relevant clinical notes
Ensures timely and accurate clinical documentation for all nursing clinical services provided
Participation in education, in-services, and other training and meetings as needed
Facilitates appropriate nursing action(s) in response to administrative directives and a changing healthcare environment
Ensures all MD orders are followed and documented accordingly during the assigned shift
Manages medication administration, monitoring, and proper storage of medication and complies with all medication management policies and procedures
