We are seeking a dynamic and experienced individual for the role of Home Health and Hospice Licensed Practical Nurse, LPN, who shares our desire to make healthcare better for everyone. Requirements
- Current Licensed Practical Nursing License in state of practice
- Ability to work independently, be self-directed, and flexible.
- Ability to use a full range of motion and ability to lift 35lbs
- Effective verbal & written communication & time management skills
- Valid Driver's License (clean record) & reliable transportation.
- Proof of current auto insurance & registration
- Valid AZ Fingerprint Clearance Card (we can help)
- Current CPR Certification (we can help) & Negative TB Test (we can test)
- All Aegis Healthcare employees must be able to pass a criminal background check.
Nice To Haves
- 1+ years of Home Health & Hospice Experience preferred
Responsibilities
- Verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.
- Providing essential nursing treatments.
- Promptly addressing patient pain.
- Assisting with activities of daily living.
- Educating both patients and caregivers on self-care techniques.
- Medication administration.
- Ensuring the availability of necessary supplies.
- Maintaining accurate patient records.
- Liaising with supervising RNs or physicians on patient assessments and any changes in conditions.
- Actively contributing to team meetings to ensure cohesive patient care coordination.
