We are seeking a dynamic and experienced individual for the role of Home Health and Hospice Licensed Practical Nurse, LPN, who shares our desire to make healthcare better for everyone. Requirements

  • Current Licensed Practical Nursing License in state of practice
  • Ability to work independently, be self-directed, and flexible.
  • Ability to use a full range of motion and ability to lift 35lbs
  • Effective verbal & written communication & time management skills
  • Valid Driver's License (clean record) & reliable transportation.
  • Proof of current auto insurance & registration
  • Valid AZ Fingerprint Clearance Card (we can help)
  • Current CPR Certification (we can help) & Negative TB Test (we can test)
  • All Aegis Healthcare employees must be able to pass a criminal background check.

Nice To Haves

  • 1+ years of Home Health & Hospice Experience preferred

Responsibilities

  • Verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.
  • Providing essential nursing treatments.
  • Promptly addressing patient pain.
  • Assisting with activities of daily living.
  • Educating both patients and caregivers on self-care techniques.
  • Medication administration.
  • Ensuring the availability of necessary supplies.
  • Maintaining accurate patient records.
  • Liaising with supervising RNs or physicians on patient assessments and any changes in conditions.
  • Actively contributing to team meetings to ensure cohesive patient care coordination.
