About The Position

The Licensed Practical Nurse (LPN) position at Viva Senior Living involves coordinating the day-to-day care of residents, ensuring staffing, and supervising direct care staff. The role requires knowledge of individual care plans and contributing to the care planning process by providing observations and information. The LPN will coordinate services with hospice, home health, and durable medical equipment companies, and manage medication administration and documentation. This position also involves responding to resident emergencies, documenting incidents, conducting community audits, and participating in resident care activities. The LPN must maintain the highest standards of care, compassion, and dignity for residents, adhering to the Resident Bill of Rights and all facility policies and procedures. The role may require being on call during exigent circumstances and fulfilling responsibilities during emergency plan activations, including assisting with resident evacuations. Confidentiality of Protected Health Information and reporting of any occupational exposures, work-related injuries, or allegations of abuse are critical aspects of this role.

Requirements

  • Must possess a valid LPN license, in good standing in accordance with laws of this state, from an approved TN school of nursing.
  • Comply with standards of business conduct in accordance with federal, state, and local health and regulatory standards and guidelines, as applicable.
  • Must possess, at a minimum, a high school diploma or a GED.
  • Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
  • Must be able to read, write, speak, and understand English.
  • Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loves ones.
  • Must adhere to all facility policies and procedures.
  • Must provide annual verification of a negative TB skin test.
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

  • Coordinate the day-to-day care of residents in accordance with the service plan.
  • Ensure sufficient staffing and coordinate staff schedules.
  • Supervise RMAs and direct care staff.
  • Contribute to the care planning process by providing specific information and observations of residents’ needs and preferences.
  • Complete any state-required assessment and service plan training.
  • Coordinate services with hospice and home health to ensure they are providing services as ordered by primary care providers and outlined in the service plan.
  • Review outside provider communication and participate in wound monitoring/management per Viva Senior Living policies.
  • Coordinate services with durable medical equipment companies to ensure residents receive medical equipment needed as directed/ordered by their medical provider.
  • Coordinate medication management with pharmacies, including managing medication orders, ensuring availability of medications, medication destruction, medication cart management, and routine auditing of medication services.
  • Administer and document medication and treatments per the physician’s order and accurately record all care provided to the residents.
  • Contribute to continuity of care and effective communication by reporting care provided and changes in the resident's condition to the responsible party, primary care provider/physician, and Resident Care Director.
  • Review End of Shift Reports and Med Tech to Med Tech logs and respond accordingly.
  • Respond to resident emergencies to ensure appropriate response and follow through.
  • Document incident reports as needed in accordance with Viva Senior Living policies.
  • Conduct community audits and training as needed to ensure compliance with state regulations and company policies.
  • Participate in routine activities of resident’s care activities, including daily living and basic nursing procedures.
  • Respond to inquiries relating to requests from residents, visitors, and other personnel promptly and courteously within given time frames and established policy.
  • Communicate and interact effectively and tactfully with the residents, visitors, families, and peers.
  • Arrive to work at the scheduled time.
  • Perform other related duties as required.
  • Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
  • Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.
  • Report work related injuries and illnesses immediately to your supervisor.
  • Complete all assigned training and skills competency.
  • Participate in all life safety and emergency drills and trainings.
  • Fulfill responsibilities as assigned during implementation or activation of the facility’s emergency plan.
  • Assist with evacuation of residents in an emergency situation.
  • Maintain confidentiality of Protected Health Information, including verbal, written and electronic communications.
  • Report non-compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
  • Protect residents from abuse and cooperate with all investigations.
  • Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator.
  • Report any retaliation or discrimination to the Human Resource Director and the Compliance Officer.
  • Fulfill individual responsibility to report any suspicion of a crime to law enforcement and the state agency.
  • Perform other duties as requested.
